Who this is for: Ops/procurement staff requesting a new subcontractor vendor, and the accounting reviewers who pre-approve them.
The whole flow: submit the request → accounting pre-approves → the vendor is emailed a tokenized intake form and created in the Vendor Portal.

At a glance
- Forms tab → New Vendor Request.
- Fill in Requestor, Vendor, and Service details. Optionally attach W-9 / COI / pricing.
- Submit Request → accounting is notified and the request lands in the Vendor Requests queue as New.
- A reviewer approves (moves it to Pre-approved) or rejects it.
- On approval the vendor contact is emailed an intake form link, and a vendor record is created in the Vendor Portal.
Step 1 — Open the form
From the Forms tab, click + New Vendor Request. The form opens inline (use ← Back to Forms to leave without submitting).
Who can submit: the New Vendor Request form is limited to a submitter allowlist. If the card is disabled for you, ask an admin to add you on the Settings page.
Step 2 — Fill it out (happy path)
Fields marked \* are required. The form is grouped into four sections:
- Requestor Information — your name, role, email, and the reason for the request.
- Vendor Information — vendor name, contact name, contact email, phone.
